2022-2023 Updates
to the Reporting Tool
Users can now specify if they
are a family member of the student or a staff member of the school when making
the initial report.
Reporters who identify themselves as family members will be
asked to specify their relation to the student. Staff members can identify
their role in the school.
Additionally, witness types can be reported on the form. If
this is left blank, the system will classify them as Unknown Witness by
default.
2022-2023
Updates to Incident Tracking System
338 Forms for both family and staff members can be generated
as auto-filled PDFs. These are located just below the SSDS buttons. They are
visible for all NJ Incidents being investigated as HIB.
Two new areas have been added to the Incident page that
correspond to the 338 forms. The first area is under the Additional Info tab.
The Detailed Description of Incident area allows users to
enter a plain text summary of the reported incident with more detail that the
brief summary provided on the General tab. This information is printed on page
1 of the 338 Family form and page 2 of the 338 LEA form.
The second additional area is located under the Conclusions
tab.
Under Conclusions – Principal/Superintendent, staff members
can fill out the final questions for the 338 forms. These responses are printed
on the final page of the 338 Family form and 338 LEA form.
